Anyone else seen this? Real or staged? Looks like he's fumbled while cocking the pistol and inadvertently moved the cylinder out of alignment with the barrel.
Back in the day when we carried the Model 66, had a somewhat similar problem. If you started to pull the trigger, then let off, then resumed pulling the trigger again, the cylinder would not index correctly; the hammer would fall, but the gun would not fire.
