If you reduce your powder charge, you can get decent accuracy with a patched round ball in a fast twist barrel. Nothing is written in stone in the black powder game.

If you are sure it's loaded, use an air compressor or c02 discharger (I love mine). If you don't have acess to those, pull the nipple, go to an auto parts store and get a grease fitting threaded in...
It's probably one of two things: your barrel and extention are not indexed correctly, or more than likely, you front sight pins were drilled off center. you could take the pins out and tap the front...

try making a shot cup from a piece of paper bag. this thread pertains to bird shot, but it may work for you....
The number is 10. Anything more than 10 rds is concidered a high cap mag, I can't buy 15 rd mags in CT anymore. If I wish to keep my GI mags, I have to regiser them.

The o ring on the extrator is to help eliminate failure to eject issues. It is probaly holding the spent case in the extractor causing the case to hit the receiver and denting it. You can remove it...
